Stages of Diabetic Retinopathy According to Ayurvedic View

The first step before starting treatment of diabetic retinopathy on ayurvedic basis is a clear diagnosis on tridosha theory. This diagnosis should be done very carefully. It is possible only by an expert hand. This is because of the different clinical stages for this disease. The signs and symptoms also vary according to the clinical stages.  It also depends on many other factors such as body constitution of patient, chronicity and severity of diabetes. Professional and psychological status of the patient, work load and eye strain of the patient also play critical roles. In general, this can be divided into four stages :  1. Stage of Siro Abhishyanda where there will not be considerable ophthalmic signs and symptoms.  2. Initial stage. Here there will be very mild ophthalmic symptoms only.  Most often, both the above stages will be ignored by the patient and patient normally does not consult an ophthalmologist.  3. In third stage, typical severe ophthalmic signs and symptoms will be manifested. There will be considerable visual defects also. 4. The speaker, Dr P K Santhakumari from her experience says that number of patients searching for ayurvedic methods for treatment of diabetic retinopathy is increasing due to various reasons.  Diabetic retinopathy is an ophthalmic condition having different clinical stages and different signs and symptoms in different stages. So, the predominance of dosha (vata, pitta or kapha) also varies accordingly. On these basis, treatment also varies. There are various treatment procedures to suit the clinical condition and dosha predominance such as sirodhara, netra tharpanam etc. Likewase, there are different methods to prepare medicine to suit the treatment procedure known as oushadha kalpana. So, first of all, an apt drug should be selected. Then the method of application or treatment procedure, and then, mode of preparation suitable for the particular method of application should be selected. Dr Santhakumari introduces about the general concept of Ayurvedic management of diabetic retinopathy. This episode gives an introduction about ayurvedic concept of diabetic retinopathy. Dr Santhakumari explains that the diagnosis is actually the analysis of signs & symptoms, and clinical stage on the basis of ayurvedic theory of Vata, Pitta and Kapha. Ayurveda is NOT depending on a single terminology such as diabetic terminology for treatment. So many factors are to be considered such as body constitution (i.e. dosha prakriti of the patient), chronicity, severity and clinical stage of the disease, dietary and habitual factors of the patient, professional and mental status of patient etc. All these factors are capable of making changes in signs and symptoms. By analyzing these signs and symptoms, the vitiated dosha, i.e. vata, pitta and kapha is found out first.
